Submit a Claim with ESIS

You can submit a claim in one of three ways:

  • Online: Access the portal to report a claim if you have a username and password. 
  • By Phone: Call 1-833-603-0941 to report a claim.
  • By Email: For Workers’ Compensation claims, contact your employer’s risk management team for your organization’s reporting protocols or your ESIS Partnership Leader’s contact information. For all other claims, or if you are unsure of who to contact, call
